Fusing / Slumping Waiver Form

1. Fusing involves the handling and cutting of cold glass. By signing below, you are agreeing that
you are aware that all glass has the potential to be sharp and could cut or poke you and will
exercise caution when handling glass.
2. Gloves are available to fused glass students and it is recommended that you use them when
cutting or handling glass.
3. Masks will be provided. All participants must wear a mask when using glass powder in their
projects as the powder is fine and may become airborne when being transferred or disturbed.
4. Eye protecting is provided. It is extremely important that you wear eye protect when you are
cutting glass or are in the close proximity of someone cutting glass.
5. The fusing studio is open to students who are 10 years old and up. All students under the age of
18 must be accompanied by an adult.
6. A safety talk will be provided at the beginning of your class. Please do your best to limit
distractions and give your full attention to this information.

PARENT/GUARDIAN AUTHORIZATION:
I am the legal guardian of the participant.
I have read and understand that my minor will be participating in and around all of the elements
described above. I believe my minor has the physical, mental and emotional ability and maturity to
handle the elements described and to work with satisfactory caution and safety in, near, around and
with all tools, equipment, machinery and glass, whether hot or cold. I authorize my minor to work at the
glassblowing studio and to participate in glassblowing, fusing or flameworking. I am aware of the high
risks associated with glassblowing, fusing or flameworking and with working in a glassblowing studio
including outdoor areas; I will not hold Blackstone River Glass, LLC. liable for injury to my minor or
damage to my minor's personal property.
